Processing of personal data

Information specific to forms, patient portal and appointment management.

What we collect online

For an appointment request: name, telephone, email address, discipline, professional, date and time chosen, version of accepted policies and technical proof of confirmation. We ask that you do not enter sensitive medical information into the public form.

Why fields are required

Without identity, means of contact and selected slot, the secretariat cannot verify or confirm the appointment. The data is only used to manage demand, avoid double reservations, send useful information and ensure traceability.

Two separate confirmations

Before sending, you must confirm that you have read the privacy policy and authorize the use of the data entered to manage the request. The date, text version and minimal proof of the action are preserved. This confirmation does not replace other legal bases that may apply to the provision of care or the center's obligations.

Emails and calendar files

When the service is activated, confirmations, modifications, cancellations and reminders may be sent to the address provided. Confirmed appointment notifications may contain a file .ics compatible with Apple Calendar, Google Calendar and calendars Android. No diagnosis will be placed in the subject of the message or in the calendar.

Minor or represented patients

The parent, guardian or representative confirms that he or she is authorized to act for the person concerned. The secretariat may request additional verification before activating access to the portal.

Exercising a right or withdrawing an agreement

Contact info@medcenter.be. Withdrawal of optional consent does not affect the lawfulness of previous processing and does not delete data subject to a retention obligation.

Data collected to respond to your message

Mandatory: first name, last name, telephone number, email address, text of the message and the two confirmations relating to confidentiality and processing.

Optional: National Register number, only for the identification of a patient already registered. For general questions, leave this field empty.

The date, policy version, minimized footprint of network address and browsing environment are kept as evidence and to prevent abuse. The message and the national number are encrypted. The acknowledgment of receipt prepared by e-mail does not include either the content of the message or the national number.
